* Note: Citizens residing in Cote D’Ivoire may download passport forms from the State Department website. However, please forward them to the U.S. Embassy. Do not mail these applications directly to the U.S. Please note, Passport Forms are also available at the Embassy.

Except in emergencies, US passports are now produced at facilities in the United States. This change will increase processing time by approximately one to two weeks. Please take this extra time into consideration when applying for or renewing passports at an Embassy or Consulate, and plan your travel accordingly.

Note on Passport Photographs
Obtain one recent, color photograph, 5 cm X 5 cm (2 in X 2 in).  The photo must be full face, with a white or off-white background.  No photographs with dark glasses or head-coverings other than religious wear which shows face, ears, and hairline.

  • Photos are available near the Embassy in Riviera Golf, next to the Leader Price shopping area.

Passport Issuance in Person for Adults

First Time Passport Issuance for Adults (aged 16 and older):
Once your claim of citizenship is substantiated, you will submit your certified, sealed birth certificate or consular report of birth abroad, one photo identification card and two photos as specified above. Personal appearance before a consular officer is required.
- Complete
form DS-11

Passport Renewal in Person for Adults (aged 16 and older):
Submit your current or expiring passport and two passport photographs as specified above.
If your present passport was valid for 10 years, complete
form DS-82.

If your present passport was only valid for five years, complete form DS-11 (see Passport Issuance for Minor Children).

Lost or Stolen Passport Replacements
Must be applied for in person. You will need to present proof of identity and proof of U.S. citizenship. Proof of identity is a photo ID, such as a U.S. license. Older or expired IDs such as student ID or previous licenses are useful if current ones are not available. Proof of citizenship is an original, sealed birth certificate with a filing number or an old canceled U.S. Passport.
- Complete forms DS-11 and DS-64, Report of Lost or Stolen Passport (required).
- Complete Police Report (recommended)

Passport Amendments after September 26, 2005
Effective September 26, 2005, as an additional security enhancement, Passport Services will no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a printing error will apply for replacement passports.

If the data page needs to be corrected within one year of issuance, the individual can complete and submit form
DS-5504, the current passport, documentation of name change (marriage, divorce, etc.), and new passport pictures to the U.S. Embassy. There is no charge for this service.

If changes are required after one year of passport issuance, please follow the instructions and payment procedures for the appropriate minor or adult passport renewal.

Visa pages may still be added to valid passports free of charge. Please bring in (or send) the original passport, completed form DS-4085.

Passport Issuance in Person for Children under 16

First Time Passport Issuance for Children:
You need to submit:

• Children born in the United States must submit certified, sealed birth certificate
• parents' passports or other photo IDs.
• A completed
form DS-11 passport application.

Effective November 1, 2004, for children under 14, at the time of application, either both parents need to be present and provide unequivocal consent to passport issuance by signing the application form in front of the Consular Officer or the parent unable to attend the interview must submit a notarized copy of form DS-3053, letter of consent. Consulates will also accept letters of consent notarized by non-American notaries. Alternatively, the parent applying for the passport must submit proof of sole custody.
Children 14 and 15 can sign their own application form.


Passport Renewal for Children under 16:

Please bring the child to the U.S. Embassy and bring the following items with you:

• Your child’s previous passport
• Your child’s birth certificate if under 14,
• A completed
form DS-11 passport application.
Parents' or guardians' passports
• Two photographs taken within the past 6 months (2x2 inches or 5x5 cm)
• Family photos to show that the child in the previous passport is identifiable with the applicant may be required.

Children 14 and 15 can sign their own application form.

Fees as of February 1, 2008

Fees for Adult Passports

First adult passport or replacement for lost/stolen/mutilated passport:
$ 100 or 48,000 cfa (which includes a separate security enhancement fee of $12)
• Adult passport routine renewal fee:
$ 75 or 36,000 cfa (which includes a separate security enhancement fee of $12)

Fees for Children's Passports (under 16 years old)

$ 85 or 40,800 cfa (which includes a separate security enhancement fee of $12)
